A Proof of Loss is a policyholder’s statement which documents the details of damage or destroyed items as well as the amount of money being requested. The form is signed and sworn to by the policyholder in the presence of a notary public. The form is designed to facilitate the investigation of the claim and enable the insurer to protect its interests.

Typical information you’ll be asked in a proof of loss form includes:

  • What covered items were involved
  • The current replacement value of the property damaged or destroyed
  • Evidence of the loss
